Dismissal Forms Allows a child to be dismissed in a non-standard way. For example, walking home rather than taking the bus.	Bus Forms Sends authorize for a child to take a different bus than they normally do. For example, visiting a friend or going to the dentist. Forms must be delivered to the office before school. For questions, email to: lunchonline @ abschools.org	CORI forms are not available online. All volunteers must fill out a Criminal Offense Record Investigation (CORI) form in the Merriam Office. Please bring your license for ID.	2012-2013 After Hours and After Hours Drop In Registration Forms are located in the link for Merriam's before and after school childcare program. Print out and fill in the forms needed. Bring it with the registration fees to the Merriam Office.

Emergency Information Cards

Parents and guardians can log in to the Power School Parent Portal for On-line Emergency Information System to supply current contact information in case of an emergency. Call the Merriam Office if you do not have a username and password. Typically parents fill these cards out at the beginning of the school year.
