| Dismissal Forms allow a child to be dismissed in a non-standard way. For example, walking home rather than taking the bus.

Bus Forms authorize a child to take a different bus than they normally do. For example, visiting a friend or going to the dentist. Forms must be delivered to the office before school.

Lunch & Milk Ticket Forms allow a child to purchase lunch tickets ($2.25 each) and milk tickets (50¢ each) in quantity. Also available is an Application for free or reduced price lunch.

CORI forms are not available online. All volunteers must fill out a Criminal Offense Record Investigation (CORI) form in the Merriam Office. Please bring your license for ID.

2009-2010 After Hours Registration Form is an application for Merriam's before and after school childcare program. Fill in the form and bring it with registration fee to the Merriam Office.
