Please bring your license for ID. | 2011-2012 After Hours Registration Form is an application for Merriam's before and after school childcare program. Fill in the form and bring it with registration fee to the Merriam Office. |

Emergency Information CardsParents and guardians can log in to the Power School On-line Emergency Card System to supply contact information in case of an emergency. Call the Merriam Office if you do not have a username and password.
